North Carolina Vital Records issues certified copies of North Carolina birth certificates, North Carolina death certificates, North Carolina marriage records and North Carolina divorce records for events that occurred within the State of North Carolina. You may order copies of North Carolina vital records through VitalChek on an expedited basis.
Recent Events: North Carolina Vital Records cannot process requests for recent events. Certificates for recent events will not be available for ordering until 90 days after the event date.
Remarks: All individuals requesting a
certified copy of a record, must present/submit copies of positive
signed identification, including, but not limited to, a driver's
license, passport or other picture identification.
Types of Certifications:
Certified Copy: - A certified copy of a certificate issued by North Carolina Vital Records includes a statement certifying that it is a true copy or true abstract of the original record held by this office. It also has the State Registrar's signature and a raised Vital Records seal.
Uncertified Copy: - An uncertified copy of a certificate issued by North Carolina Vital Records will be on plain (white) paper and will be clearly stamped "Uncertified". It will not have a raised Vital Records seal.
Remarks: A certified copy is required for most legal or official purposes for which a copy of a vital record is requested.
P.O. Box Drop off Form - Send the completed form with $24 money order, certified check, or business check (not a personal check) for the first copy of the certificate and $15 for each additional;
By Mail - You can go through the steps for mail-in service but choose to pay more for faster processing and/or faster delivery. Write "Expedite" on the outside front of envelope;
In-Person - You can go through the steps for drop-off service but choose to pay more for same-day, walk-in service;
Other Options - Vital event certificates may be obtained more quickly at the Register of Deeds (ROD) office in the county where the event took place;
VitalCheck - Secured online applications to request for vital records, which accepts all major credit card payments, can be made through VitalCheck with an additional charge for their service.

State law restricts access to the registrant's Immediate Family members or those who represent tangible proof of legal interest in the requested record.
Immediate Family means any of the following: mother, father, sibling, child, current spouse, or maternal or paternal grandparent. Paternal grandparent is eligible if father is listed on the vital record.

Paternity Determinations
A paternity acknowledgment is a legal action that determines the biological parentage of a child. Paternity may be established through voluntary acknowledgment by the father and the mother or by court order.

Legitimation
A legitimation is a legal process for biological fathers to acknowledge their children when they marry the mother after the birth of the child.
A new birth certificate may be issued reflecting the father's name and the child's name can be changed to the father's last name if the parents agree and request the change.

Court-Ordered Name Changes
An amended birth certificate reflecting a name change ordered by the courts may be processed for individuals who are born in North Carolina.
Please complete the birth certificate application form providing the information to locate the original birth certificate.

Birth Certificates after Adoption
Certified copies of an adopted person's birth certificate are available only to the adoptee, the adoptive parents, the adoptee's children, and the adoptee's spouse, brothers, and sisters (proof of relationship may be required).
Original birth certificates of adoptees showing the adoptee's original name and the birth mother's name are in legally sealed files and are not available.

Changes to Death Certificates
After acceptance for registration by the State Registrar, no death certificate can be altered or changed except by a request. The State Registrar adopts rules governing the form of these requests and the type and amount of proof required.
Original birth certificates of adoptees showing the adoptee's original name and the birth mother's name are in legally sealed files and are not available.
