The Maine Division of Vital Records (Maine Vital Records) issues certified and non certified copies of Maine birth certificates, Maine death certificates, Maine marriage records, and Maine divorce records for events which occurred within the State of Maine from 1923 to the present.
Remarks: All requests must include an
unexpired copy of Government or State-issued Photo ID, and the
completed & notarized request form with signature of the person
requesting the certificate.
How to order a certified copy:
By Mail - Written request through the mail with payment by check or money order. We do not accept credit cards through the mail. Please enclose a long, self-addressed, STAMPED envelope with your request;
Walk-In - Walk-in request with payment by cash, check, or credit card. Vital Records is located at 244 Water Street in Augusta, Maine;
VitalCheck - Secured online applications to request for vital records, which accepts all major credit card payments, can be made through VitalCheck with an additional charge for their service.
