The Illinois Department of Public Health preserves the state's records on births, deaths, marriages, civil unions and dissolutions. If the event took place in Illinois and you are seeking a copy of the record, the county and circuit clerks may have the document on file.
Remarks: All requests must include a
non-expired copy of Government or State-issued Photo ID and
signature of the person requesting the certificate.
Amendments and Corrections
Illinois law allows for corrections to vital records. In most cases, additional documentation and a state of Illinois affidavit and certificate of correction are required.
Remarks: Corrections can be requested by either a parent listed on the birth record if the child is a minor, the child if older than 18 years of age or a legal guardian.
How to order a certified copy:
By Mail - Current processing time on mailed requests for "Certified Copies Only" is approximately three to four weeks from the date the request is received.;
Expedited service requests will be processed within three to five business days of receipt
By Fax - Orders can be faxed to 217-523-2648 all necessary documents including the completed application fro search of birth record files;
These orders may take three to five business days to process. Incomplete or illegible applications will not be processed. An adult signature is necessary to accept the UPS delivery.
In-Person - In-person orders can be dropped off for mail out within two business days at the Illinois Department of Public Health, Division of Vital Records office, from Monday through Friday from 10 a.m. to 3 p.m., excluding holidays.;
VitalCheck - Secured online applications to request for vital records, which accepts all major credit card payments, can be made through VitalCheck with an additional charge for their service.

Birth records can be obtained from the Illinois Department of Public Health (IDPH), Division of Vital Records or from the county clerk's office in the county where the birth occurred.

Commemorative Birth Certificates
The Vital Records Division of the Illinois Department of Public Health offers a personalized, Birth Certificate for only $40.00. The Commemorative Birth Certificate makes a great gift, especially when it's engraved with baby's name and birth date.
Remarks: Commemorative birth certificates are not available for individuals whose records were filed prior to 1916.
The Illinois Adoption Registry and Medical Information Exchange (IARMIE) program provides a means by which registrants may authorize or prohibit the release of identifying information, including a copy of the adopted person's original birth certificate, to others involved in their surrender or adoption.
Remarks: Confidential facts may be released to registrants only after at least two specified parties to the adoption have filed explicit mutual consent for the exchange of this information.
A certified birth record is essential for foreign travel purposes. It can be used to provide proof of citizenship when applying for a passport or used instead of a passport for short-term travel to some countries.
Remarks: If your child is younger than 16 years of age and the birth certificate will be used for a passport application, the passport office requires the $15.00 certified copy.
When biological parents marry after a child is born, a legitimation process must be completed. The Affidavit by Biological Mother, Affidavit by Biological Father, certified copy of the marriage record and fee are required.
Remarks: The fee is $15 and includes one certified copy of the birth certificate. Additional copies ordered at the same time are $2 each.
If the biological parents of a child are not married, are both available to sign forms and no other man is listed on the birth record, a voluntary acknowledgment of paternity form is completed.

You must have complete gender reassignment surgery before the "Affidavit by Physician After the Completion of Gender Reassignment" and the "Affidavit for a New Birth Certificate After Completion of Gender Reassignment Surgery" can be completed and submitted to this office.
Remarks: The $15 fee to create a new birth record due to gender reassignment includes one certified copy of the new birth record. Additional copies of the same record requested at the same time are $2 each.
Illinois law created a process for establishing a parent-child relationship between a child and his or her intended parents when the child is being carried by a gestational surrogate.
Remarks: This relationship must be established prior to the birth of the child in accordance with Section 12 of the Vital Records Act and Section 6 of the Illinois Parentage Act of 1984, and the Gestational Surrogacy Act.
