Certified copies of vital records are available from the Department of Health or the Vermont State Archives and Records Administration (VSARA).
Remarks: All requests must include a copy
of State-issued or Government-issued Photo ID of the person
requesting the Certificate. Expired cards or documents will not be
accepted.
How to request for vital records:
- You may order as many copies or different certificates as you want on one purchase order.
- Each certified copy costs $12 ($10 statutory fee plus a $2 on-line service fee).
- Most orders will ship via standard US mail within 5 to 7 business days of your request.
- Expedited shipping is available at an additional charge based on your location. Expedited orders will be shipped overnight on the day your order is fulfilled.
- If your order can't be fulfilled, you will receive a partial refund (you pay only the search fee and the on-line service fee for any copy ordered but not shipped).

accepts all major credit card payments, can be made through
VitalCheck with an additional charge for their service.

document Vermont state government from its creation in 1777 to the

registered properly and that the information on the certificate is
correct. Any changes or corrections after six months may require a
court order.
