Looking for Birth Certificate from Antwerp

I have a partial photograph of the birth certificate for Frederic Auguste Cokaynen born in Antwerp, Belgium on August 2 or 3 in 1896. It appears to be written in Dutch. I would like to obtain a full certificate and have it translated into English. His parents are listed as Edward Francis Phillips Cokayne and Sara Annie Skates who are from London, UK. I am his grand-nephew.
